Applications to My Place Germantown must come from one of the following:

City Outreach • Shelters • Transitional Housing

Applications must document recent homelessness, a certified disability,
and evidence of capacity to live independently.
The target population is males between 45 and 60 years of age.

Applications are available by contacting Hans Reiser, Program Director, at 215-848-2892.

Completed applications must be submitted to the Philadelphia Office of Supportive Housing for review,
with a copy sent to My Place Germantown, attention Hans Reiser.

Additional review will be done by the Philadelphia Redevelopment Authority
and the My Place Germantown staff.
